This form is used in the Incoming U14 - U18 age groups as our "go ahead" to contact you to talk about the 2018 JO season. This is simply the 1st step toward evaluating a possible match between us. These conversations are confidential and there is no assumption that you will leave your current club or that we can offer you a spot at Elevation.

Players who will be in the U10 - U13 in 2018 should wait for our TRYOUT REGISTRATION FORM to appear in late summer and should not use this form. We will also be holding tryouts in the U14 age group and for selected spots in the U15-U18 age groups.
